For over 150 years dermatologists have sought to do things to the *surface* of the skin in order to treat the *depth* of the skin.
1) Deep chemical peels: involve strong acids that eat through the *surface* of the skin in order to treat the *deep* skin
2) Full wire brush dermabrasion scrubs away the *surface* of the skin in order to treat the *deep* skin.
3) CO2 laser resurfacing blasts away the *surface* of the skin in order to prompt tightening of the *deep* skin.

Yes, the surface of the skin is traversed with an array of 25 needles with each "shot", but the needles just pass in and out.
for it is at the *tips* of the needles that the treatment occurs.
through the *tips* of the needles bipolar radio-frequency energy is deposited into the *depth* of the skin ... to achieve tightening, scar remodelling, rejuvenation.
Using insulated needles, all the treatment energy gets deposited in the deep dermis, leaving the superficial skin/epidermis barely affected beyond a few needle-pricks.
